Technical Assistance Packages Instructions & Applications

General Information

The PRC-HAN Conference Planning Workgroup is pleased to offer three Technical Assistance Implementation Packages to build on learnings from the recent PRC-HAN Research to Practice Symposium and Georgia Mental Health Forum, May 19-20, 2008, Atlanta, Georgia. Implementation Packages are available for three evidence-based depression care management programs for older adults:

The organization and delivery of these start-up packages for an implementation-ready agency will vary for each site, but may include private, professional consultation, initial needs assessment, delivery/licensing of program materials, site visits, and telephone follow-up. Implementation Packages are valued at $3,000 each. Awarded agencies will only be required to cover economy travel costs associated with site visit consultation (if applicable to the individual package).

Instructions & Requirements

  1. Implementation Packages will be awarded through a competitive application process based on an assessment of organizational readiness and fit between interest, resources and capacity.
  2. Interested organizations are asked to complete an Application Form (below) for the Implementation Package(s) of their choice.
  3. Applications must be typed, submitted by the deadline, adhered to the instructions, and complete to be considered.
  4. Applications are to be submitted electronically as an attachment to Lesley Steinman, UW Health Promotion Research Center, lesles@u.washington.edu. (IMPACT applicants will need to fax their Implementation Grid to Lesley as well at 206-543-8841). Please indicate “Implementation Package Application” in the subject line.
  5. Deadline for Application submission is Monday, July 7th, 2008 at 5pm PST. Please note that applications received after this date will not be considered. Applicants will be notified by Monday, July 28th, 2008.
  6. Implementation Package Awardees must agree to: a) designate one key person that has primary responsibility for the Implementation Package; work with the TA team to finalize the Implementation Package for your organization; b) initiate program implementation activities by September 26, 2008; and c) complete and return a two-page interview report one month following the package completion (no later than March 30, 2009).>
  7. PRC-HAN Conference attendance is not required for applicant organizations; however, proposals that involve individual(s) that attended the conference will be given special consideration for the packages.
